How does Forms work?

Collect private responses, registrations, feedback, and applications.

How do I build a form?

Open Forms and create a blank form or start from Feedback or Application. Add up to 20 questions, choose answer types, and mark required answers. Preview uses the same controls members will fill in. Conditional questions can depend on a choice in an earlier question. Save your draft before publishing.

What should I know about publish and share?

Publish & open makes the saved draft available to server members. Copy its link into Discord or a Message Studio link button. Editing a draft does not change the live form. Each response keeps the questions it was submitted against. Close responses stops new submissions; archive also moves the form out of your current list. Restore returns it as closed.

How do I choose access and response limits?

Discord sign-in and server membership are always required. Optionally require any one of your chosen server roles. Collect responses records answers without a decision. Review mode lets staff accept or reject each response. Choose one response every 90 days or one every 24 hours. Identity is attached to every response. Do not ask for passwords or secrets.

How do I read and review?

Staff Access controls form management, response reading, reviewing, and deletion independently. These permissions apply across the server. Open Responses, filter by status or Discord user, and read a submission. Decisions and notes are visible to the member when they reopen the form link. Another reviewer must handle your own response. Export this page downloads up to 50 responses with their original question labels and answers.

What should I know about retention and limits?

Responses expire after 90 days, including accepted and rejected ones. Staff with deletion access can permanently remove an individual response. Exported copies remain outside Yuna. Servers can have up to 50 forms, including archived forms, and each form can retain up to 10,000 responses. Use Tickets when a request needs an ongoing conversation, and Appeals for moderation decisions.
